• The "Parental-Guardian Overnight Travel Consent Form" has to be filled and signed for each retreat participant.

  • indicate in the form below the preference for receiving link(s) by email to an online platform where the form can be filled and signed electronically.

Important

Participants will be denied access to the retreat if the Parental-Guardian Overnight Travel Consent Form is not properly filled, signed and delivered to the parish offce before the date of departure.

Payment for the retreat has to be made before departure as well.
